Dutasteride is prescribed off-label for hair loss, and it has a slow onset because of its long half-life (~5 weeks) and the biology of hair growth cycles. Most patients will not see visible results until 3-6 months. Initial shedding often occurs in months 1-3 and is a positive sign. Regrowth is maintained only while treatment continues: once the drug clears, hair loss resumes.
Month-by-Month Timeline
Month 1: DHT Suppression Begins
Serum DHT levels begin falling within the first week. Scalp DHT reduction follows more slowly. No visible hair changes are expected at this stage. Serum dutasteride reaches 65% of steady state after 1 month and about 90% after 3 months, so the drug builds up slowly.
Months 1-3: The Shedding Phase
Many patients experience an initial increase in shedding. This occurs because weakened, miniaturized hairs in the telogen (resting) phase are pushed out as the follicle re-enters anagen (growth) under the influence of reduced DHT. Months 3-6: Early Regrowth
Shedding typically stops and early signs of regrowth appear. New hairs may initially be thin and vellus-like before progressively thickening. Hair loss stabilization becomes noticeable.
Months 6-12: Visible Improvement
Most patients see noticeable improvement in hair density and thickness. Existing hairs become thicker as follicle miniaturization reverses. Scalp coverage improves.
Months 12-24: Peak Results
Hair count, thickness and density continued to improve to the end of every published randomised trial, all of which stopped at 24 weeks or 6 months. Reports of continued gain through 12 to 24 months come from clinical practice rather than from controlled trials. 1-year results data.
Patience Is Critical
Discontinuing dutasteride before 12 months is the most common mistake. Many patients who perceive the drug as “not working” at 3-4 months go on to see significant improvement by month 8-12.
