Dutasteride (brand name Avodart) is FDA-approved for the treatment of symptomatic bph/">benign prostatic hyperplasia (BPH) in men with an enlarged prostate. It is also widely prescribed off-label for androgenetic alopecia (male pattern hair loss), female pattern hair loss in postmenopausal women, and has been studied for prostate cancer chemoprevention in the REDUCE trial.
FDA-Approved Use: Benign Prostatic Hyperplasia (BPH)
Dutasteride’s only FDA-approved indication is the treatment of symptomatic BPH. It reduces prostate volume by a mean of 24.7% at 12 months and 26.7% at 24 months, improving urinary symptoms and reducing the risk of acute urinary retention and BPH-related surgery. Off-Label Use: Male Pattern Hair Loss
Dutasteride is increasingly prescribed off-label for androgenetic alopecia. Multiple clinical trials have demonstrated that dutasteride 0.5 mg daily produces superior hair regrowth compared to finasteride 1 mg daily, likely due to its more complete DHT suppression (a median serum DHT reduction of about 94% at one year, against about 65% suppression for finasteride 1 mg). Off-Label Use: Female Pattern Hair Loss
Dutasteride is used off-label in postmenopausal women with female pattern hair loss (androgenetic alopecia). Because dutasteride is contraindicated in pregnancy and can cause abnormalities of the external genitalia in a male fetus, it is only appropriate for women who cannot become pregnant. Research Use: Prostate Cancer Prevention
The REDUCE trial (Reduction by Dutasteride of Prostate Cancer Events) studied dutasteride as a prostate cancer chemoprevention agent. The trial found a 22.8% relative risk reduction in biopsy-detected prostate cancer over 4 years. The FDA did not approve dutasteride for this indication: the label carries the limitation of use that dutasteride is not approved for the prevention of prostate cancer, and records a higher incidence of Gleason 8-10 prostate cancer on dutasteride (1.0%) than on placebo (0.5%). Topical Dutasteride
Topical dutasteride formulations (0.01-0.05%) are an emerging treatment option for hair loss, delivering the drug directly to the scalp with potentially fewer systemic side effects. A 2025 Phase II trial reported increases in hair count at these concentrations. Full topical dutasteride guide.
