Twice-weekly dutasteride dosing (0.5 mg taken two days per week) is an off-label protocol that relies on the drug’s terminal half-life of about 5 weeks to hold DHT suppression while cutting total exposure. In a 2025 randomised pilot study of 60 men, twice-weekly and thrice-weekly dutasteride both increased hair density and diameter over 24 weeks. Dutasteride is not FDA-approved for hair loss at any dose or frequency.
Why Twice-Weekly Dosing Works
Dutasteride has a terminal half-life of approximately 5 weeks, by far the longest of any commonly prescribed hair loss medication. This means that even after skipping several days, serum dutasteride levels and DHT suppression remain significant. Serum concentration therefore falls slowly between doses rather than cycling sharply, which is the pharmacokinetic rationale for the reduced-frequency schedules. The 2025 pilot study did not report serum dutasteride or DHT levels, so the size of that difference has not been measured in this setting.
Clinical Evidence
Sereepanpanich and colleagues randomised 60 men aged 21 to 60 to dutasteride 0.5 mg twice weekly, dutasteride 0.5 mg thrice weekly, or finasteride 1 mg daily for 24 weeks. Hair density and diameter rose significantly in all three groups. Mean change in terminal hair count was 7.74 hairs/cm² on twice-weekly dutasteride, 17.43 on thrice-weekly dutasteride and 12.81 on daily finasteride. Thrice-weekly dutasteride beat twice-weekly, but did not significantly beat daily finasteride on hair count. Sexual adverse events were similar across the three groups. Note what the trial did not include: there was no daily-dutasteride arm, so it cannot tell you what a reduced-frequency schedule costs against taking dutasteride every day.
